Working Together and Sharing Successes:
By working together, member organizations in PCCY help to identify gaps in services to children and youth and reduce duplication of effort of agencies by serving as a forum for the sharing of information. PCCY also works to improve the delivery of services by meeting and developing a common understanding among the organizations involved. Here are some of the recent efforts PCCY members have worked on:
Family Assistance Coordinated Services and Family Connections
Mendocino County Social Services Branch (SS) Family Assistance Coordinated Services (FACS) project is funded by a two-year grant from the California Center for Research on Women and Families (CCRWF). The purpose of the project is to coordinate services provided by the Employment and Family Assistance Services (EFAS) Division, which is responsible for CalWORKs, Medi-Cal, Food Stamps, and CalWORKs Job Services, the Family and Children’s Services Division.
Case conference teams will develop coordinated case plans for families that are involved in both divisions. Originally a stand-alone project, the FACS project is joining Family Connections, a collaborative program that is funded by the Mendocino County First 5 Commission. Family Connections is a multidisciplinary, coordinated system to serve families with children prenatal to age 5 and link them with preventive services, parent education and support, and mental health, substance abuse, and health care services in the greater Willits area
